Please nominate someone who has permission by a parent/guardian to collect the child from The Hub For Kids. Please note the servicereserves the right to refuse authorisation of a minor or others in order to protect a child from risk or harm. Incase of emergency or illness, if we are unable to contact you, your authorised nominees will be contacted to act on yourbehalf and may give authorisation for emergency medical, hospital, ambulance, dental or administration of medical treatment.
Every reasonable care is taken to ensure your child’s well-being while s/he attends The Hub For Kids, however, in an emergency situation we require your permission to respond quickly to any accident that may occur. In the event of an accident or illness requiring emergency medical, hospital, ambulance or dental treatment, every effort will be made to contact you.
However, should this prove impossible, please indicate permission to respond to emergency by signing below. I authorise the approved provider, nominated supervisor or educator of the children’s service to seek emergency medical, dental and hospital treatment, via ambulance service or private transport for my child, should this be necessary.
I will ensure my child arrives on time for their session and is collected on time. Sessions will begin promptly, and a late fee of $10 will apply for every 10 minutes I am late in collecting my child. I understand that if my child does not attend because of sickness or for any other reason, that I need to contact The Hub For Kids.
If my child is sick or absent more than once in a term, I will be required to pay for the missed sessions to secure his/her position. I am aware that if I know ahead of time that my child will be absent, I can contact The Hub for Kids to try to schedule a make-up session.
My child may attend The Hub For Kids only once enrolment forms are completed and payment is made. An invoice will be sent out to me via email prior to the commencement of each month. No refunds can be made once payment is processed. It is my responsibility to ensure The Hub For Kids has my current email address.
If I choose to cancel my child’s enrolment, a written letter/email is required with a minimum of two weeks notice. I am liable for any fees owing up until the end date.
Public holidays are not charged, therefore, monthly fees are set accordingly. In the event that the teacher is absent and no replacement can be found, the fee for that session will either be fully refunded or credited to the following month’s fees. Your child may receive a home reader. Please take good care of our readers as they are a valuable resource. If a reader or resource is lost or damaged, a charge may apply to cover the cost of its replacement
I have read and understood all the terms and conditions of enrolment and agree to abide by these policies. I will notify The Hub For Kids immediately if any of the details provided change.
I understand that I am not permitted to bring my child when s/he is not well. This includes when s/he has a fever, heavy cold or cough, vomiting or diarrhoea.
I understand that if my child requires a medical plan, a copy must be provided to The Hub For Kids. It must be current and approved by a doctor.
I consent to my child’s first name and photograph being used for promotional purposes or to be displayed in the room.
The Hub For Kids only accepts payment by direct deposit. Bank details are included on your term invoice.
As the sessions offered at The Hub For Kids are quite short, no food or snacks are required. A clearly labelled drink bottle (water only) is permitted. All stationary supplies will be provided by The Hub For Kids.
